  35. This is the document whereby you, while living and competent, make
  36. desired changes to provisions of your trust.  Changes in final
  37. beneficiaries, the shares each will receive, successor trustees, or
  38. the age of distribution are examples of revisions that might be
  39. required due to changes in your circumstances.

  47.                      AMENDMENT TO TRUST AGREEMENT

  49. A.  JOHN M. SMITH, Trustor, executed a Trust Agreement dated (*insert
